Offsite:
Services are primarily provided in-office. However, clients who meet at least one of the following conditions may be eligible for off-site services:
- Have significant physical disabilities/limitations without access to transportation
- Have a medical condition and are requesting services in a hospital or other medical setting
- Have either physical or mental health symptoms that would be exacerbated by any travel
- Have a significant mental health issue, which prevent them from being able to leave their home
